Create doctor account

To create doctor account follow these steps

• From this page you can create new doctor account
• Enter the center name
• Enter the center username
• Enter the email
• Enter phone number
• Type a password
• Re-type the password
• Check the agree button
• Press the sign up button to create the doctor account
